Social Worker Job Responsibilities 

Assessment of care situation, including:
*Assesses clients social and emotional needs related to illness or injury, need for care, response to treatment and adjustment to care including the relationship of the medical and nursing requirements to the home visit situation monthly, quarterly, or as need it
*Assess, plan, implement and evaluate the psychosocial services for patients
*Special needs related to cultural diversity including communication, space, role of family members and special traditions 
*Patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect.
*Identifies family dynamics and communication patterns 
*Environmental resources and obstacles to maintaining safety 
*Maintains ongoing outreach with community agencies in order to identify agency services and act as liaison between agency and the community 
*Provides in-service education on social work related topics as needed/requested 
*Prepares clinical records 
*Participate in the marketing efforts of the office as assigned 
*Participates in community relations activities as directed by the agency Management 

Provides social services, including:
*Provide family education and counseling 
*Provide direct supervision to caregivers
*Provider training to caregiver per regulations
*Developing crisis intervention plans and implementing plans
*Serving to meet the social, emotional and other psychological needs of our clients.
*Collaborating with nursing staff and other professionals to meet clients’ needs.
*The Social Worker plays a vital role in promoting a homelike environment for our clients.
*Helps coordinate and guide all aspects of our clients experience from admission to end-of-life care
*Assists nursing staff in addressing and tracking medication reduction efforts and alternative therapies such as request P/T, O/T, R/P
*Meets state and federal regulations for social services tasks regarding resident care
*Coordinates conferences requested by families, residents, or staff
*Assistance in providing information and preparation of advance directives and funeral planning issues 
*Transfer of responsibilities regarding fiscal, legal and health care decisions 
*Assists the physician in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness 
*Participates in the development and revision of the plan of care 
*Prepares clinical and progress notes 
*Identifies and utilizes appropriate community resources and support systems 
*Educates patients and families related to social service issues, including long term placement, financial issues
*Assesses bereavement needs 
*Participates in discharge planning 
*Provides educational programs to staff and community.

*Ability to prioritize and manage a caseload within a rapidly changing, high activity and crisis prone environment
*Function well under pressure, independently 
*Ability to effectively cope with patients, families and all others with varying backgrounds, socioeconomic conditions, and value judgments 
*Ability to assess safety of home situations for self, patients, and other staff members, including physical and psychological dangers 
*Ability to provide counseling and support to all age groups impacted by disabilities
*Ability to foster and maintain professional working relationships with community agencies, and staff
*Maintains confidentiality of information relating to patients and staff
*Additional Requirements at the time of hire must provide: Social Work Degree/ and or a license

*Qualifications 
A Social Worker must have at least a bachelor’s degree in social work from a school of social work accredited by the Council on Social Work Education. A Master degree is preferred.
*A qualified Social Worker will have a current license in the state of Minnesota, with one (1) year of social work experience in a health care setting.
*Experience with the Medicare program and participation in community agency activities is preferred.
*A valid driver’s license and use of an insured auto or access to adequate transportation is required
*Oriented and educated in culture, gender, and age
*Proficient with Microsoft Office products including Word and Outlook, Spreadsheet, Power Points
*Experience in a healthcare setting preferred 
*Experience working with an Electronic Medical Record system preferred
